Description:
Under supervision of a Clinical Pharmacy Manager as designated, the clinical pharmacist will function as a clinical expert in the area of anticoagulation within a comprehensive telephone-based anticoagulation program.

Essential Responsibilities:
  • Specific clinical pharmacy service responsibilities may include: assessing new member medication regimens, patient enrollment into specific services, educating patients and/or caregivers regarding drug therapy and disease management, coordinating drug therapy with home health care services, telecommunication with patients and practitioners, notifying patients regarding dosage adjustments, scheduling lab follow-up appointments, designing drug treatment plans for patients, managing adverse events and drug interactions, using computerized pharmacy databases for patient tracking and documentation and generating consultations to practitioners or notes for medical record entry.
  • Comprehends and utilizes policies, procedures, protocols and TSPMG treatment guidelines as appropriate for the clinical pharmacy service.
  • Develops and collects data for quality improvement projects.
  • Provides pharmaceutical care for members including prospective, concurrent and retrospective drug utilization review, screening for drug allergies, monitoring interactions and duplications, following-up with physicians as appropriate, and counseling patients.
  • Interacts and consults with practitioners when necessary to improve quality of care and improve value; educates practitioners regarding cost effective therapy and drug utilization goals (e.g. preferred drug, formulary and drug promotion).
  • In conjunction with supervisor, orients and instructs new pharmacy personnel regarding departmental policies and procedures. Provides drug-related training for pharmacy and other Health Plan employees and practitioners.
  • Participates in patient education programs and coordinates development of educational materials as appropriate.
  • Serves members in a friendly, courteous and expeditious manner to promote member satisfaction and retention.
  • Performs general pharmacist duties when needed; maintains efficient inventory control and purchasing habits when needed.

  • Pharm D or Bachelor's of science in pharmacy.
  • License, Certification, Registration
  • Georgia Pharmacy License.

  • Additional Requirements:
  • Motivated to expand pharmacy knowledge base and comfortable researching drug topics.
  • Complete a Customer Service Assessment.
  • Previous experience in specific clinical pharmacy service areas (e.g. anticoagulation, drug benefit and formulary management) preferred.
  • Knowledge of formulary systems, pharmacy operations, pharmacy benefit management, inventory control systems, cash control and budgetary procedures for a high volume pharmacy.
  • Customer service aptitude demonstrated through Customer Service Assessment.

  • Post graduate year residency training, preferred, not required.
