Works with the Clinical Pharmacy Services team to provide patients, providers and support staff counseling and evaluation to ensure optimum drug therapy at maximum cost-effectiveness to member; and provides new and current members with continuous medications where appropriate. Makes members/patients and their needs a primary focus of one's actions; develops and sustains productive member/patient relationships. Actively seeks information to understand member/patient circumstances, problems, expectations, and needs. Builds rapport and cooperative relationship with members/patients. Considers how actions or plans will affect members; responds quickly to meet member/patient needs and resolves problems.

Essential Responsibilities:
  • This position, knows and complies with all Kaiser Permanente quality, safety, and emergency policies and procedures. Demonstrates quality and effectiveness in work habits and clinical practice in every interaction with patients, colleagues, providers, and leadership. Ensures patient safety in the preparation and provisioning of care related to but not limited to medications, procedures, infection prevention, fall prevention, including consistent use of two patient identifiers and procedural time outs. Reports safety hazards, accidents and incidents, and unsafe working conditions promptly.
  • Interviews selected patients; records medication history on Pharmacy Consultation form. Assesses patient compliance with medications; counsels patients to achieve optimal use of medications. Monitors patients for real or potential problems with drug and/or substance abuse or misuse.
  • Reviews medication profiles of selected patients prior to medical appointments. Makes recommendations regarding drug therapies to provider using clinical knowledge and target drug program.
  • Answers or appropriately routes pharmacy refill requests for absent providers when patient chart is available. Resolves pharmacy refill request problems to prevent member inconvenience when possible.
  • Provides drug information service for patients, providers and support staff using clinical knowledge or Clinical Pharmacy Services team.
  • Assesses physician prescribing trends of assigned area through use of the Pharmacy Information Management system and through communication with Pharmacy Supervisors and Clinical Pharmacy Services team.
  • Assists new members with medication needs prior to first appointment by arranging for transfer of existing prescriptions from outside pharmacies or obtains new prescription authorization from provider.
  • Performs other duties and accepts responsibilities as assigned, such as working with Pharmacy Supervisors at locations without a Clinical Pharmacist in clinical programs.

  • BS in Pharmacy or PharmD
  • License, Certification, Registration
  • Currently licensed as a Registered Pharmacist in the State of Colorado.
  • National Provider Identifier (NPI) and Taxonomy code required at time of hire.

  • Additional Requirements:
  • Must demonstrate the ability to problem solve.
  • Interpersonal, written and verbal communication skills.
  • Ability to critically demonstrate the ability to problem solve.
  • Ability to critically analyze drug literature and lay press to provide drug information to health professionals and members.
  • Must display commitment to Target Drug Programs and cost-effective prescribing initiatives.
  • Demonstrated clinical pharmacy knowledge and high motivation to seek continuing education to keep abreast of current drug therapies.
  • Must possess PC skills in word processing.
  • Must authorize a D.E.A. drug screening.
  • Demonstrated customer service skills, customer focus abilities and the ability to understand Kaiser Permanente customer needs.

  • Preferred Qualifications:
  • One (1) year of experience as a Registered Pharmacist preferred which includes counseling patients regarding appropriate use of medications (including herbals, vitamins, noncompliance issues, adverse drug reactions, side effects, etc).
  • Prefer previous experience as a Kaiser Permanente Pharmacist or as a Clinical Pharmacist in a similar professional environment.
