The Sleep Case Manager works collaboratively with sleep physicians and primary care providers to provide case management services for members with sleep disordered breathing on sleep therapies. By meeting the patients comprehensive sleep health needs the Sleep Case Manager promotes quality health outcomes to effectively manage a patient's sleep disorder.

Essential Responsibilities:

  • Conducts face to face, group and/or telephone visits and performs physical, psychosocial and educational assessments and develops, implements, evaluates and documents a member guided care management plan outlining sleep study results, sleep therapy device education and follow up. Determines effectiveness of device therapy-treatment plan and the member's abilities, limitations and barriers to self-management. Works toward the goal of achieving optimal sleep therapy self-management and compliance.

  • Reviews the diagnostic sleep testing results entered in Epic by the sleep physician's sleep study interpretation and provides results and education to patients.  Schedule follow up appointments with patient based on physician interpretation-plan of care.

  • Performs data downloads from sleep device and reviews report data providing usage information from machine and compliance based on Medicare requirements.  Knows when to apply critical thinking to engage sleep physician to report symptoms, persistent treatment issues, other potential medically related issues or barriers to optimal care.

  • Establishes and maintains communication with other team members as well as with sleep physicians(s) and/or other referring physician(s). Maintains appropriate documentation in KP Health Connect, and other information systems.

  • Provides education on alternative therapies for OSA as necessary.

  • Educate and custom fit new patients with PAP equipment.  Ensure thorough understanding by the patient to enable compliance with therapy.  Maintain an effective rapport with patient to facilitate compliance with procedures. Troubleshoots sleep therapy device and interfaces. Determine appropriate course of action such as, mask fitting, desensitization or other equipment interface based barriers. Maintains data on members, utilization and results of the program to measure quantitative and qualitative outcomes.

  • Sets up patients to Home Sleep Study equipment and takes a detailed sleep history. Process all data obtained from sleep studies and analyze data.  Prepare standardized report for physician review.

  • Performs other duties as assigned.

  • Basic Qualifications:


  • Minimum one (1) year Sleep DME equipment experience or minimum two (2) years in lab titrating experience.

  • Minimum two (2) year experience in a polysomnography lab.

  • Minimum two (2) years' experience with diagnostic monitoring.

  • Education

  • Associate's degree in health care or related field OR two (2) years of experience in a directly related field.

  • High School Diploma or General Education Development (GED) required.

  • License, Certification, Registration

  • Licensed Respiratory Care Practitioner (LRCP) in Oregon and Washington (must have current license in state assigned; secondary license due within 6 months of hire) AND Respiratory Therapist Sleep Disorder Specialty credential (CRT-SDS or RRT-SDS) within 6 months of hire; OR Registered Polysomnographic Technologist (RPSGT) AND Oregon Licensed Polysomnographic Technologist (LPSGT) at hire.

  • Basic Life Support (BLS) for Healthcare Providers at hire.

  • Additional Requirements:

  • Advanced knowledge of cardiopulmonary physiology related to respiration and sleep.

  • Polysomnography & proficient sleep scoring.

  • Knowledge and operation of all monitoring systems utilized in sleep lab.

  • Courteous, clear and effective oral and written communication with clinicians, nurses, healthcare providers and patients.

  • Ability to travel to various Sleep Clinic sites across the region.

  • Demonstrated experience of advanced sleep therapy equipment.

  • Preferred Qualifications:

  • Minimum two (2) years' experience with Sleep computer-based diagnostic systems and programs.

  • Minimum two (2) years' Sleep based clinical patient experience.

  • Minimum one (1) year experience with Ambulatory Home Sleep Studies.

  • Minimum two (2) years' experience with Sleep based DME equipment program.

  • Oregon Licensed Polysomnography Technologist (LPSGT).

  • Licensed Respiratory Care Practitioner (LRCP) with SDS (Sleep Disorders Specialist) endorsement.

  • Attended Clinical Sleep Educator training or CCSH Certified.

  • Formal polysomnography training.
