Under the direction of the CQSS Prevention Services Manager, and in collaboration with relevant medical specialty departments, provide and maintain centralized outreach, medical support, and management for identified members of high-risk populations.



Essential Responsibilities:

  • Primary and secondary prevention of disease. Review lists of patients in high-risk populations. Review chart to assess accuracy of diagnosis. Assess for follow up testing using regional algorithms for guidance. Telephonic and written communication with members. Perform health maintenance services and patient education including counseling members on importance of nutrition, exercise, fall prevention, etc. Follow-up on test/screening results. Order pharmacologic therapy as appropriate. Member medication review. Assess barriers to staying on treatment(s). Counsel and educate members to overcome barriers. Communicate with PCPs regarding evaluation results and treatment plans. Refer patients for specialty consultations for difficult problems. Document in writing all care in patient's permanent medical record in accordance with accepted medical/legal standards. Provide all medical care in accordance with accepted practice standards, hospital/clinic protocols (including supervision by a physician), and State licensing guidelines.

  • Coordinate, develop and organize patient or department services or programs. Examples include, but are not limited to: Assist in developing protocols. Screen incoming referrals. Coordinate educational activities for department. Develop written patient education materials. Provide in-service training. Chair or participate on committees.

  • Performs other duties as assigned.

  • Basic Qualifications:

    Experience

  • Minimum two (2) years managing large volume of patients.

  • Education

  • Graduate of AMA-approved PA or ANA-approved NP training program (both provide master's or equivalent degrees) or equivalent requirements to meet State licensure.

  • High School Diploma or General Education Development (GED) required.

  • License, Certification, Registration

  • Graduate of AMA-approved PA or ANA-approved NP training program.

  • PA: National certification required at time of hire. Current Oregon AND Washington state medical license required. License in state where assigned required upon hire/transfer, application for license in secondary state required within 2 weeks of hire/transfer and must be obtained within 6 months. Prescribing registration required at time of hire.

  • NP: National certification required at time of hire. Current Oregon AND Washington RN license required. License in state where assigned required upon hire/transfer, application for license in secondary state required within 2 weeks of hire/transfer and must be obtained within 6 months. Current Oregon AND Washington state NP license required. License in state where assigned required upon hire/transfer, application for license in secondary state required within 2 weeks of hire/transfer and must be obtained within 6 months. Prescribing registration required at time of hire.

  • National Provider Identifier (NPI) required prior to employment start date.



  • Additional Requirements:

  • Thorough and working knowledge of all common diseases in specialty field; familiarity with less common diseases.

  • Working knowledge of State health care laws, licensing board regulations and controlled substance regulations. Important to know limitations and when to refer patients.

  • Ability to assess, diagnose and effectively treat common medical disease and injuries.

  • Ability to manage large volume of patients, and to interface with large provider and support staff.



  • Preferred Qualifications:

  • Minimum five (5) years of clinical experience.
