Community Health Program Specialist

Responsible for working closely with other members of the Community Health team to coordinate programs and partnerships that address health and social needs in communities, and to represent KP in the community. Supports collaboration across community sectors and KP departments to advance KP's community health strategy. Contributes to efforts related to grant making and monitoring, and support of the KP grant portfolio, including communication, evaluation and reporting processes. May participate in collaboratives, committees and planning groups. Programs that this role supports relate to health care access, economic opportunity, and neighborhood and school health and wellness.

Essential Responsibilities:

  • Program Coordination: Coordinates programs and partnerships that supports KP's community health strategy. Supports programs through coordinating communications, relationship building, collaborative problem solving, accessing technical assistance for partners, tracking project progress and stakeholder convenings. May participate in grant development, review, processing, management and site visits.
  • Stakeholder Coordination: Builds and sustains relationships across multiple internal departments and community sectors. Supports strategic partnerships with community, government and other stakeholders to address determinants of health related to health care access, educational attainment, economic opportunity, built environment, medical and social services and other areas. Works with Community Health managers to support program development and implementation, event planning and sponsorship, community engagement and other efforts. Assists in coordination and support of meetings, internal and community events, workshops, and grantee convenings. Assists community organizations inquiring about and responding to funding opportunities.

  • Minimum four (4) years of related experience.


  • Bachelor's degree in community health, community relations, community organizing OR four (4) years of experience in a directly related field.
  • High School Diploma OR General Education Development (GED) required.

  • Proven success at building and maintaining relationships with community-based organizations, community coalitions/collaboratives, and networks.
  • Solid written and oral communication skills that demonstrate cultural competence and ability to work effectively with a wide range of internal and external stakeholders.
  • Existing and emerging knowledge of best practices in organizing and implementing community health policies/programs. Proven project management skills, and ability to maintain high productivity amid competing priorities.
  • Proven ability to exercise sound judgment under pressure.
  • Ability to prioritize and manage own work; exercise independent judgment, initiative and motivation. Proven ability to follow through to produce deliverables.
  • Demonstrated skills in problem solving in a complex environment, and ability to work effectively with diverse stakeholders.
  • Ability to interact professionally and effectively in a wide variety of public settings when necessary.
  • MS Excel, MS Word and MS PowerPoint skills required.

    Preferred Qualifications:

  • Four (4) years of relevant experience in communications, process/project work or community relations preferred.
  • Experience coordinating initiatives with diverse stakeholders.
  • Experience in grant proposal review and/or writing.
