Description:
Collaboration with physicians, extenders, nursing, ancillary staff, medical records and coding staff to maintain the integrity of the patient's story through documentation. Compliantly improves upon the documentation through designated processes maintaining coding guidelines and facility bylaws. Supports timely, accurate and complete documentation of the clinical information used to measure and report physician and facility outcomes. Educates all members of the patient care team on an ongoing basis.

Essential Responsibilities:
  • Identifies and coordinates possible modifications to clinical documentation and quality standards, which reflect an accurate and complete record of the patients encounter.
  • Demonstrates knowledge of DRG issues, documentation opportunities, clinical documentation requirements and referral policies and procedures. Ensures the accuracy and completeness of the clinical information used for measuring and reporting physician and hospital outcomes.
  • Updates the CDI Review electronic worksheet in a logical and clear fashion to communicate to others the clinical findings in the record and the associated coding concepts. Educate all internal customers utilizing a pre-defined education and delivery structure that cover: Clinical Documentation Opportunities, Coding and Reimbursement issues, as well as Performance Improvement Methodologies.
  • Conducts follow up reviews to ensure points of clarification have been recorded in the patient's chart. Reviews clinical issues with coding staff to assist in assigning a final DRG. Tracks response to CDI Queries and trends completion of the final DRG. Maintains thorough and current knowledge of clinical care and treatment of assigned patient populations to critically assess appropriateness of documentation.
  • Maintains proficiency of CDI practices, and keeps apprised of new regulatory and documentation requirements. Performs other job duties and responsibilities as required. Follows Department CDI processes to improve quality measures and drive outcomes while adding input for continuous process improvement.
  • Basic Qualifications:
    Experience
  • Minimum two (2) years of clinical experience in a hospital setting, or one (1) year recent clinical experience and one (1) year ICD-10 coding experience.
  • Education
  • Graduate of a school of nursing or Registered Health Information Technologist (RHIT), Registered Health Information Administrator (RHIA).
  • License, Certification, Registration
  • Certified Documentation Improvement Practitioner (CDIP) or upon hire, work towards certification (within 3 years).
  •  
    Additional Requirements:
  • Demonstrated knowledge of and skill in decision making, analytical thinking, customer service, influence, interpersonal relations, oral communications, problem solving, project management, quality management, results orientation, systems thinking, teamwork, and written communication.
  • Demonstrated knowledge of and skill in word processing, spreadsheet, and database PC applications.

  • Preferred Qualifications:
  • Prior experience in clinical documentation improvement, utilization review/management, quality management, case management or coding.
  • Bachelor's degree in nursing.
  • Graduate of school of nursing ad Registered Health Information Technologist (RHIT), Registered Health Information Administrator (RHIA), or Certified Coding Specialist (CCS).
  • Completion of an accredited Health Information Management program.
  • Working knowledge of the KPHC (Epic) system.
