Supervisor Ambulatory Social Medicine - Honolulu - FT - Hybrid
Under the direction of Continuing Care or Ambulatory Social Medicine Manager, establishes and maintains, standards, quality levels, cost effective, and customer focused ambulatory social medicine services. Establishes and interfaces with community services. Provides ambulatory social medicine services which add value and are linked to strategic business and organizational goals and objectives.
- Responsible for the overall supervision of ambulatory social medicine services and staff - includes interviewing, hiring, communication of goals, objectives, accountabilities and priorities, monitoring assignments and evaluation of personnel; develops / presents / coordinates in-services to ensure staff competencies and on-going training.
- Provides direct patient care on an as needed basis. Provides services that are within scope of license and in compliance with all legal and regulatory requirements.
- Collaborates/consults with physicians, departmental leaders, other departments, other users of ambulatory social medicine to ensure services provided are integrated and comprehensive.
- Assures coverage of ambulatory care needs during regular operations and after hours. Participates in social medicine policy and procedures review and updates. Identifies community resources.
- Minimum four (4) years of experience in health care setting.
- Minimum one (1) year management or leadership experience.
- Masters degree in Social Work.
- Licensed Social Worker (Hawaii) OR Licensed Clinical Social Worker (Hawaii)
- Basic Life Support within 1 months of hire from American Heart Association
- Demonstrated knowledge of and skill in adaptability, change management, conflict resolution, decision making, customer service, influence, interpersonal relations, oral communication, negotiations, problem solving, results orientation, teamwork, team building, written communication.
- Working knowledge of state and federal health care programs.
- Knowledge of the principles of patient teaching and delegation, care planning, and appropriate utilization of acute hospital, long term care, and home care resources.
- Demonstrated ability to produce and present management reports.
