Job Summary:

In addition to the responsibilities listed below, this position is also responsible for overseeing and guiding teams in the provision of and/or providing rehabilitation services (e.g., physical therapy, occupational therapy, speech therapy) care plans across the continuum of care; coordinating and evaluating rehabilitation services (e.g., physical therapy, occupational therapy, speech therapy) daily operations; maintaining accountability for rehabilitation services teams clinical competency in evidence-based care and compliance with all licensure and documentation requirements; leading and guiding the development and implementation of rehabilitation services clinical initiatives and training and education programs; and leading collaborations with interdisciplinary teams and physicians on the clinical program development and implementation of rehabilitation services clinical practice and shared models of care.

Essential Responsibilities:

  • Provides developmental opportunities for others; builds collaborative, cross-functional relationships. Fosters open dialogue amongst team members, engages, motivates, and promotes collaboration within and across teams. Delegates tasks and decisions as appropriate; provides appropriate support, guidance, and scope; encourages development and consideration of options in decision making.

  • Manages designated work unit or team by translating business plans into tactical action items; oversees the completion of work assignments and identifies opportunities for improvement; ensures all policies and procedures are followed. Aligns team efforts; builds accountability for and measuring progress in achieving results; determines and ensures processes and methodologies are implemented; resolves escalated issues as appropriate; sets standards and measures progress. Fosters the development of work plans to meet business priorities and deadlines; obtains and distributes resources. Removes obstacles that impact performance; identifies and addresses improvement opportunities; guides performance and develops contingency plans accordingly; influences teams to execute in alignment with operational objectives.

  • Demonstrates clinical expertise by: implementing and/or guiding teams across multiple sites and/or a single site in the implementation of clinical standards, protocols, and complex, long-term projects based on the latest research and industry developments to improve workflow efficiencies and patient outcomes; contributing to the development and implementation of evidence-based practice standards across multiple sites and/or a single site, and monitoring and evaluating related outcomes and providing training to ensure adherence to evidence-based and medically necessary standards; providing training and ensuring that teams across multiple sites and/or a single site have the necessary resources, training, and support to effectively provide care and educate others (e.g., members, families, caregivers, the heath care team) on industry developments and evidence-based best practices; and overseeing participation in appropriate and approved educational and clinical development opportunities (e.g., quality improvement activities, continuing education courses, staff training) to maintain and enhance competencies in professional practice for teams across multiple sites and/or a single site.

  • Leads teams in maintaining a safe and compliant care practice by: coaching teams across multiple sites and/or a single site to comply with federal, state, and local regulations, ensuring departmental policies and procedures align with external requirements; providing guidance on the development of reports for administrative and regulatory purposes and disseminating insights from reports across multiple sites and/or a single site; proactively identifying and mitigating potential risks to patient, staff, and member safety by implementing safety protocols; implementing safety-related policies and procedures and ensuring teams across multiple sites and/or a single site to report safety hazards, accidents and incidents, and unsafe working conditions, overseeing implementation of corrective actions to avoid future incidents and contributing the development of new policies and procedures; and accountable for ensuring that teams maintain confidentiality and security for all patient, staff, and member records and information across multiple disciplines and/or locations.

  • Leads efficient and effective operations management by: managing allocation of resources (e.g., personnel, equipment) across multiple sites and/or a single site,implementing processes to streamline operations and maximize cost savings; optimizing and streamlining processes of stocking shelves, taking inventory, ordering supplies, and cleaning equipment to enhance efficiency and cost-effectiveness; analyzing performance metrics to assess effectiveness of services across multiple sites and/or a single site and guiding teams to implement data-driven solutions to enhance service delivery (e.g., quality, efficiencies, cost); and contributing to and facilitating strategic planning, staffing, budget, and performance discussions for multiple sites and/or a single site, collaborating with cross-functional teams to achieve organizational objectives.

  • Leads patient care practices by: conducting and/or overseeing teams across multiple sites and/or a single site to conduct comprehensive assessments of patient condition and needs; developing, and/or coordinating and monitoring the development and implementation of patient care plans across multiple sites and/or a single site, proactively identifying and addressing complex patient needs, goals, and preferences; leveraging extensive knowledge of patient safety and care practices to manage the provision of care across multiple sites and/or a single site and ensure safe administration of medications, procedures, and/or infection control; guiding teams to determine medical necessity and need for continued care based on rate of progress; promoting patient advocacy across multiple sites and/or a single site, encouraging team members to continuously assess treatment efficacy to identify needs for treatment modifications; overseeing modifications to treatment/care plans and mentoring team members across multiple sites and/or a single site on evaluating complex responses to treatment and developing modifications to care plans for complex cases; and overseeing discharge processes across multiple sites and/or a single site for patients who have reached established goals or received maximum treatment benefits.

  • Leads service quality improvement initiatives by: managing staff and projects across multiple sites and/or a single site to improve the quality, safety, and efficiency of therapy services and coordination of care services; managing the resolution of complex service/care issues, unusual occurrences (UOs) including patient complaints/grievances, and guiding teams across multiple sites and/or a single site to resolve complex patient concerns; guiding teams across multiple sites and/or a single site to identify and implement process improvements to increase access, improve efficiency, and promote quality outcomes of therapy services based on data analysis and feedback from patients and staff; collaborating within the team and with leaders across multi-disciplinary teams to foster care coordination, improve service delivery, and elevate the patient experience; and developing new products and services that align with company goals, member needs, and emerging evidence and trends in health care, ensuring implementation of new products/services by teams across multiple sites and/or a single site.
Minimum Qualifications:


  • Masters degree in Physical Therapy, Occupational Therapy, or Speech Therapy AND minimum four (4) years of experience in clinical rehabilitation therapy or a directly related field.

  • Minimum three (3) years of experience in a leadership role with or without direct reports.



  • Occupational Therapist License (California) required at hire OR Physical Therapist License (California) required at hire OR Speech-Language Pathologist License (California) required at hire
  • Basic Life Support required at hire
  • National Provider Identifier required at hire
  • PECOS Enrollment within 6 months of hire
