Job Summary:

Coordinates, facilitates and manages 24-hour staff visit scheduling process of the Continuing Care Services Department for Home Health and Hospice, Home Infusion Nursing, Palliative Care, and Physical Medicine to meet identified operational objectives in a manner consistent with department policies, union contracts, operational efficiency and mission. Makes possible the delivery of most effective, timely, quality care to the homebound patient.


Essential Responsibilities:

  • Manages staffing/fulfillment/patient needs of the CCS Department by coordinating resources with the needs of the CCS Department by scheduling staff for home visits in a manner that makes possible maximum staff productivity and cost control. Matches patient needs and patient location with staff availability, skill, workload, union seniority and geographical assignments. Initiates supervisor contact to augment or reduce staff as necessary. Acts as a resource to supervisors, managers, directors, and support staff to meet informational needs. Analyzes, assesses, projects, and produces accurate and timely staff schedules to ensure appropriate staff/patient coverage. Follows appropriate regional and departmental policies and procedures, Local 49 and OFN union contracts. Effectively maintains daily accurate computerized records of admissions, discharges, visits needed, visits scheduled, staff availability and skills, visit allocation system points, and statistical data. Prepares daily statistical reports. Facilitates staff/patient visit scheduling that minimizes professional staff overtime and mileage, outside referrals and unnecessary visits while maximizing staff productivity and operational efficiency.
  • Facilitates communication between physicians, patients/families, department supervisors and managers, Home Health/Hospice, Home Infusion, Palliative Care, and Physical Medicine staff using pagers, written messages, computers and voice mail. Effectively communicates with clinical supervisors to coordinate staff/patient visit needs. Makes decisions without prior approvals, which include creating solutions, problem solving/negotiating, conflict resolution, and setting personal work priorities. Informs appropriate staff of patient discharge or death in a timely manner.
  • Participates in departmental quality management activities including performance measurement of scheduling processes.
Basic Qualifications:

Experience


  • Minimum two (2) years of experience in a staff scheduling position or equivalent knowledge/experience.

  • Per the National Agreement, current KP Coalition employees have this experience requirement waived.

  • High School Diploma.


License, Certification, Registration

Additional Requirements:


  • High School Diploma or General Education Development (GED).

  • Working knowledge of personal computers, database software and adapt them to meet departmental needs.

  • Conflict resolution/negotiation skills.

  • Basic mathematical skills.

  • Geography of northwest Oregon and southwest Washington.

  • Basic knowledge of a Health care delivery system.

  • Demonstrates customer-focused service skills.

  • Able to analyze complex problems.

  • Able to manage multiple tasks and conflicting demands for service.

  • Organizational, patter recognition and process analysis skills.

  • Skills in problem solving/critical thinking and prioritization to meet a fast-paced, ever-changing environment.

  • Able to make and communicate decisions and take action in high-pressure situations with tact, diplomacy and respect.

  • Demonstrated effective communication skills and strong attention to detail.

  • Ability to accurately process a large quantity of data despite distractions and interruptions.

  • Demonstrated excellent attendance record.

  • Equivalent experience in a closely related field (customer service, computerized scheduling, scheduling software, patient-based computer application, dispatching, accounting).


Preferred Qualifications:


  • Two (2) years of recent experience in scheduling or closely related field.

  • Home Health/Hospice scheduling experience; Two to three (2-3) years of experience working with Kaiser Permanente operations.

  • Previous experience in design, coordination and maintenance of staff schedules.

  • Thorough knowledge of home health/hospice and Kaiser Permanente operations.

  • Working knowledge of HMO philosophy and mission, computerized scheduling systems/software, voice mail, word processing, database software, productivity measurements and continuous quality improvement. Working knowledge of KARE system.

  • Thorough knowledge of union (Local 49, OFN) labor contracts, and home health/hospice operations.

  • Medical Terminology.

  • 10-kay calculator skills.

  • Associates degree in communications, bookkeeping or accounting or related knowledge and experience.
