Educator - Clinical Nurse Specialist - Case Management Team - Renton WA
EDUCATOR - CLINICAL NURSE SPECIALIST - CASE MANAGEMENT TEAM - INPATIENT, ER AND ADVANCED URGENT CARE DISCHARGE PLANNING
Monday- Friday, rotating weekend on-call every 6th weekend
Provides in-depth clinical expertise in a defined area of nursing practice; e.g., wound care, oncology, geriatric care, critical care/emergency, behavioral health, hospice, etc. Develops and ensures quality standards that are evidence based for nursing practice to a specific patient population. Provides advanced clinical practice education, consultation, research and administration. Participates in and may lead components of staff orientation, skill assessment, development and implementation of specific clinical curriculum for nursing staff. May be responsible for leadership, development, monitoring and implementation of quality and population based care programs.
Essential Responsibilities:
- Establishes and monitors care standards and makes effective recommendations for changes in clinical practice. Collaborates with nursing staff in assessing patient, family age-specific nursing needs and develops plan of care for complex, high-risk patients. Evaluates the effectiveness of care. Makes recommendations to change standards of care or standards or practice based on research. Defines and implements best nursing practices. Provides consultation to physicians, nurses and other health care providers in meeting care needs of individual or groups of patients in outpatient and/or inpatient settings.
- Assesses staff knowledge and identifies competency-based educational needs of staff. Designs and implements educational programs to ensure high quality staff knowledge and skills. Introduces new techniques/innovation based on advanced practice trends. Staff assessment authorityincludes recommendation to managers regarding need for corrective action of individual staff. Provides input into performance evaluations regarding clinical competence and provides coaching and counseling. May assist in interviewing and in some situations, supervising staff.
- Designs teaching programs for patients/families and provides assistance to nursing staff in program implementation. Develops or leades development of interdisciplinary plans of care for complex and high risk patients.
- Participates in and consults in setting the direction for quality performance and improvement standards, monitoring and evaluating outcomes. May collect and analyze data regarding patient care outcomes, trends, morbidity andmortality. May have other responsibilities unique to the defined area of nursing practice including the provision of direct patient care, writing grant fund proposals, department liaison, etc.
- Minimum three (3) years of recent clinical experience.
- Masters degree in Nursing or other healthcare related field.
- Registered Nurse License (Washington) required at hire OR Compact License: Registered Nurse required at hire
- Basic Life Support required at hire
- Clinical expertise in area of specialty.
- Negotiation, facilitation, presentation and computer skills.
- Program development, evaluation, individual skills assessment and adult learning theory.
- Clinical nurse specialist experience in area of specialty.
- Wound care certification or other certification specific to the area of clinical practice. (e.g., wound care, PALS, ACLS), preferred by date of hire/transfer.
- Prior public speaking and knowledge of Kaiser Foundation Health Plan of Washington or other health care system.
- Quality and process improvement methodology and translation into clinical practice.
