Outpatient Pharmacist - Riverfront, Spokane

- Completes work assignments by applying up-to-date knowledge in subject area to meet deadlines; follows procedures and policies, and applies data and resources to support projects or initiatives with limited guidance and/or sponsorship. - Supports drug education and training efforts by: planning and scheduling individual trainings to continue drug education and staff development in-services for pharmacists, physicians, and other health care professionals, and students.
- Supports pharmaceutical patient care and drug therapy by: developing productive relationships with care providers and members; providing simple case-specific drug information (e.g., drug therapy, adverse effects, compliance, appropriate use, and handling) to health care providers and members, minimal day-to-day supervision; following designated drug therapy needs of standard patients and/or monitoring progress of therapy through interviews, physical assessment, patient education and clinical laboratory monitoring, with limited guidance; collecting, analyzing, and presenting simple therapeutic outcomes to health care providers to help identify possible improvements in drug plan, with general directions; tracking and reporting utilization, as appropriate, of targeted medications and medication classes to support the evaluation impact of initiative work to assure safe, rational and cost-effective prescribing; identifying and analyzing clinicians decisions, support, and feedback using tools, effectively engaging support for the drug use management process; and monitoring and coordinating standard pharmaceutical care and optimal utilization of resources to and from acute and ambulatory patient-care settings.
- Plans and schedules contributions for internal and external quality, safety, and emergency policies, standards, and procedures by: ensuring all activities complies with all regulatory rules and regulations (e.g., Drug Enforcement Administration [DEA], State Board of Pharmacy, Health Insurance Portability and Accountability Act [HIPAA] and relevant internal regional and national policies; ensuring patient safety in the preparation and provisioning of care (e.g., medications, procedures, infection prevention), with limited guidance; reporting safety hazards, accidents, incidents, and unsafe working conditions; and collaborating with physicians, other medical professionals, and health plan personnel to provide information about and assure quality and process outcome measures.

- Pharmacist License (Washington) required at hire
- National Provider Identifier within 6 months of hire
- Advanced Practice Pharmacist (APh).
- One (1) year of experience working in a large matrixed organization.
