Job Summary:

Develops, implements, and evaluates regional clinical pharmacy programs for patients, providers, and pharmacists. Maintains a clinical pharmacy practice to impact patients at high risk for drug-related problems in area of expertise. Educates providers, pharmacists and other health care professionals. Makes members/patients and their needs a primary focus of ones actions; develops and sustains productive member/patient relationships. Maintains knowledge of and assures departmental compliance with Kaiser Permanentes Principles of Responsibility and policies and procedures, and applicable regulatory requirements and accreditation standards.

Essential Responsibilities:

  • This position, knows and complies with all Kaiser Permanente quality, safety, and emergency policies and procedures. Demonstrates quality and effectiveness in work habits and clinical practice in every interaction with patients, colleagues, providers, and leadership. Ensures patient safety in the preparation and provisioning of care related to but not limited to medications, procedures, infection prevention, fall prevention, including consistent use of two patient identifiers and procedural time outs. Reports safety hazards, accidents and incidents, and unsafe working conditions promptly.

  • Develops and maintains a clinical pharmacy practice that includes: Drug therapy consultations for members and healthcare providers. Participates in drug use initiatives. Provides continuing education and staff development in-services for pharmacists, physicians, and other health care providers. Provides drug information to health care providers and members. Reviews, critically, medical literature to provide updates on drug therapy.

  • Participates in local, regional and interregional committees as appropriate.

  • Precepts pharmacy students and residents.

  • Develops and participates in research activities.
Basic Qualifications:

Experience

At least one (1) of the following: PGY2 residency or fellowship (Board Certification required within three (3) years of employment) OR PGY1 residency and a minimum three (3) years of clinical experience (in related field) and board certification OR no residency and a minimum five (5) years of clinical experience (in related field) and board certification.


Education


  • Doctor of Pharmacy degree.


License, Certification, Registration


  • Pharmacist License (Colorado) required at hire



  • National Provider Identifier required at hire


Additional Requirements:


  • Board certification required within three (3) years of employment if previous experience includes PGY2 residency or fellowship.

  • Board certification required immediately if previous experience does not include PGY2 residency or fellowship.

  • Taxonomy code required at time of hire.
